We haven't been there in years, but when we did go, absolutely loved it. We stayed on the small island (Perhentian Kecil) at a place called D'Lagoon on one of the northern-most bays. The guesthouse was a bit of a dump, but the snorkelling was awesome. There was also a back beach that you could walk to via a jungle trail -- it remains probably the most beautiful beach I've ever seen in Asia.

You may like to check out the website :
www.perhentianguide.com .
Also Redang Island which is nearby is a place worth checking out .The first time I was there, 20 years ago, there were no hotels and we have to pitch tents and we sighted a a boatful of vietnamese refugees making landfall one morning . Pulau Redang was then near to Pulau Bidong, a Vietnamese Refugees holding center pending repatriation to a 3rd country.
Best time to visit would be April/ OCt as the North -East monsoon is between Oct to May .
You will need to get yourself to Kota Baru if you are going to Perhentian and to Kuala Trengganu if going to Redang . AirAsia flies daily to both these two places.
You will not regret it.
