My partner and I are in our 30/40s and live in Auckland, New Zealand. From Bangkok onwards we'll be meeting family and traveling with them.
Any advice is appreciated. Also, will the weather be okay in all the places we'll be going to at the times we are going? Is this the best route? Is it realistic?

A couple of thoughts:

I'd take a day off Siem Reap and give it to Phnom Penh
You're time on Koh Rong will be at the tail end of wet season -- there will be some rain.
From Phnom Penh you can do 2/3 night trips via the Mekong Delta that will deposit you in Saigon. While they are a bit of a touristic experience, for a quick look at the Delta, they can be a good option. Any travel agent in Phnom Penh will be able to assist in this regard.
No airport at Hoi An -- you'll be flying to Da Nang and then getting a transfer south to Hoi An. (minor point)
I'd take a day off Nong Kiaow and give it to Luang Prabang.
Vientiane deserves another day.

Thanks for the feedback. So take a day off both Hanoi and Siem Reap you reckon, and add another day to Phnom Penh and Vientiane?
My main concern is the weather. Will it be okay? North Vietnam sounds okay at that time of year, but I'm worried the rest of the places especially Hoian and Koh Rong Island. Has anyone travel to these places at that time of the year?

There is always a risk of bad weather in that period. Just be flexible.
For Hoi An it isn't such a problem since there are things that can be done indoors and things can still be interesting.
Koh Rong might not be that pleasant if you're unlucky but then you can cut that short and spend more time in Phnom Penh. With good weather you can stick to your itinerary.

Weather is definitely the watch word for Cambodia at that time of year. 2012 was not too bad in terms of rain, but previous years have seen a lot of flooding in September and October. You will need to be prepared to be flexible with travel plans and if you plan snorkelling or diving, the water can be very cloudy if the rains do tank it down.
If you are relaxed and chilled out and can chop and change your itinerary you'll be OK! And of course we could get a rainy season like last year when everything just worked as normal!
