Itinerary and route for 3 weeks in Thai, Cambodia, & Vietnam

    My wife and I are going to Thailand (briefly), Cambodia and Vietnam for 3 weeks in late June and early July.
    We land in Bangkok and plan to go to Koh Chang, then on to Cambodia (Phomh Penh and Angkor Wat) and Vietnam. I had hoped to get as far as Hanoi and Halong bay, but now I am thinking this is too much ground to cover.
    Is there a easy/safe/best route to cross into Cambodia from Koh Chang ? Am looking for best route and order to see Angkor wat, Pnomh Penh, Chau Doc, Saigoin, Hoi An etc. Is Phu Quoc worthwhile ? Best, swimmable waterfall/rive enroute ?
    Any itnerary suggestions and comments would be appreciated. Our interests are culture, temples/ruins, waterfalls, beaches and sunsets.
