Booking ahead?

I will be traveling in Thailand and Vietnam from about Xmas through mid-Januray, with plans of going to Bangkok, Kanchanburi, and Ko Chang in Thailand and hanoi, Halong Bay, and Sapa in Vietnam. I know that will be a busier time since it is over the holidays-- how much do I need to book ahead? I'd rather play it by ear when I get over there, but I don't want to be left without accomodation!

Airfare

Is it better to book flights within country (Thailand) and between countries (Thailand/Vietnam) on-line before I leave, or through a travel agency/on-line once I'm over there? Will the prices be much different?

What are the best airlines to use?
