Any way to avoid transfers in BK for this? We were going to overnight VIP bus to Chang Mai. Then thinking maybe overnight train back to BK and then decide if we transfer to bus or continue on with train.

Love some suggestions!

No, you'll have to pass through Bangkok. Heading south, bus is the better bet as the train won't take you all the way to Phuket (rather you'll have to get he train to Surat Thani and then a connecting bus from there).

Another option is to fly -- check AirAsia's website for cheap tickets from CM to BKK and then onwards to Phuket.

Thai Air has direct flights from Chiang Mai to Phuket but they aren't particularly cheap.
