Is it better to book flights within country (Thailand) and between countries (Thailand/Vietnam) on-line before I leave, or through a travel agency/on-line once I'm over there? Will the prices be much different?
What are the best airlines to use?

Depends where you're going. Look into Bangkok AIrways Discovery Air Pass -- it can only be purchased overseas and is a very good regional deal. See the FAQ for links to various airline websites.
