From Kep , the road to the border is a dirt road. We managed fine in a car (not sure about tuk-tuks).

At the border you will get your Cambodia exit stamp and either walk (10 mns) or hire a moto to the Vietnamese side to get a stamp (no VOA).
Then it's a 10 mn ride to Ha Tien .

We hired a private taxi from Rith Travel - he has an office at the Kep Crab Market - a few doors down from the LP recommended Kimly crab restaurant.
He will ask for $35 dollars for the journey from Kep to Ha Tien. He will drive you to the border, arrange a moto and then another car on the other side. It all went very smoothly and only took an hour over all.

The border crossing in itself is fine and no bribes were asked.

In Ha Tien there is a ferry going to Phu Quoc at 13.00 for 160,000 Dong.
There was an 8.00 ferry with Cashin but this one has been in maintenance since September so there is now only the one ferry at 13.00. We never found a Vinashin office (it seems this one only goes from Rach Gia) but the Cashin office instead (they will sell you tickets fro the 13.00 ferry with a different company).

We've not yet been on the ferry so can't report yet on the quality of the crossing to the island.
