How about this for a rough outline on the "mainland": start in Bangkok and make your way to southern Thailand. Finish with the islands or in Krabi. Fly to Phnom Penh. Go to Siem Reap by bus or fly. In addition or instead of Sihanoukville, I would highly recommend Kampot. Beautiful, modern, charming town in the south on a river with day trips possible. Then cross by land or fly to southern Vietnam (Ho Chi Minh) and go up the coast of Vietnam (bus or train or plane). Then cross into Laos by land or plane (to Vientiane). Then fly or cross by land into northern Thailand (Chiang Mai).
You can fit Borneo and other destinations either in between the destinations above, or at the end of your trip around the "mainland".
