There are no trains in Laos or anywhere north of Chiang Mai. Boat transport is only between Luang Prabang and the border at Huay Xai. So most of the way will be by bus or minivan. See travel info on http://www.hobomaps.com

I like the overland route by bus or minivan as Bob mentions, because you can make a stopover at Vang Vieng. But you could also look into flying to Luang Prabang from Vientiane on Lao Airways.
Depending on where you travel before heading to Laos, you could also fly directly to Luang Prabang from other locations too. You can also fly directly from Luang Prabang to other places, including Chiang Mai.

You can also fly that route with Lao Central Airlines (http://www.flylaocentral.com/), which was quite a bit cheaper when I did it a couple years ago. The bus ride is long and winding; if going that way you might want to break it in half by spending a night or two in Vang Vieng.
